How to Create a Custom Form

 

First, go to the following address and login to your StudioPro account.

https://app.studiopro.uk/admin/authentication/login

 

It is important that when you try to login to your admin account that you have the /admin/ at the end of www.studiopro.uk

 

Example: www.studiopro/admin or www.studiopro/yourtag/admin

 

IMPORTANT: If you’d rather not read the process then you can either watch how to do this or when you login to your account there is a “Tour Guide” which you can use to follow the process.

 

Step 1: Create Form from Scratch

 

  1. Use the left sidebar to navigate to Studio Manager > Flex Form Builder
  2. Click the “New Form” button.
  3. Modify the form fields to match anything extra you require.
  4. Click the “Design and Configuration” button at the top right.
  5. Scroll down to “Form Settings” and turn on:
    1. Make sure that the “End Date” is in the future before publishing.
    2. Set who should be notified if form is submitted.
  6. Click the “Save Changes” button at the bottom right.
  7. Finally, click “Publish” button at the top right corner.

 

 

Step 2: Integrate to your website

 

  1. While editing the form click “Share” button at the top right.
  2. Either copy the link and add to a button on your website or copy the iframe code and embed on your website.

 

Once you have completed either route for creating a PARQ form, this form will now appear in the class booking process and will be required before class booking is complete.
